Sakuta's first love, the girl who reached out to help Sakuta during Kaede's incident in the past, after which her whereabouts became unknown. Sakuta decided to apply for Minegahara High School because she attended it.
She should currently be 19 years old, but for some reason, she appears as a 12-year-old first-year middle school student.

The original world line before any changes. In this world line, Shoko Makinohara in the fourth grade failed to complete her "Future Dreams" assignment due to her teacher's words, causing Puberty Syndrome and creating two personas: "Shoko who wants to grow up" (Little Shoko) and "Shoko who doesn't want to grow up" (Adult Shoko). In Sakuta's view, she (Adult Shoko) enrolled in Minegahara High School and spoke to Sakuta, who had escaped from the hospital ward, on Shichirigahama beach, saying"Thank you," "You did your best," "I love you" — live bravely with these three phrases in your heartThis encouraged the dejected Sakuta and changed his outlook on life, thus making her his first love. Two years later, Little Shoko found the abandoned cat Hayate and met Sakuta and the others. She temporarily entrusted the cat to Sakuta and visited his home regularly. Later, Sakuta learned that she suffered from a severe congenital heart disease. AtKaede AzusagawaAfter regaining his memories, Adult Shoko appeared again to comfort Sakuta. She "lived together" with Sakuta for several days while Mai was away filming a movie, successfully creating a love triangle drama when Mai returned. Adult Shoko revealed that she was the Shoko from the future and that she existed to fulfill her younger self's dreams. Later, while trying on an off-the-shoulder wedding dress at the Morito Coast Chapel, Sakuta discovered the transplant surgery scar on her chest. She explained to Sakuta—just as Futaba had predicted—that due to the theory of relativity, Adult Shoko had reached the future ahead of time[1], but she traveled back in time[2], and further disclosed the fact that she survived by receiving a heart transplant. After the date ended, Adult Shoko deliberately chose the same Christmas Eve date location as Mai—to see the Enoshima illuminations—so Sakuta decided to go to the aquarium for his date with Mai that day. Sakuta's condition later flared up again, and finally, Adult Shoko revealed the fact that Sakuta would be involved in a car accident on December 24th while heading to his Christmas Eve date with Mai, and his heart would be donated to her.[3]Additionally, Shoko traveled back to save Sakuta because Sakuta, due to Adult Shoko's influence, visited Little Shoko every day and said"Thank you," "You did your best," "I love you" — live bravely with these three phrases in your heartthereby becoming Little Shoko's first loveThis is a chicken-and-egg problem, so Shoko hopes Sakuta can find happiness with Mai. Additionally, this explains Sakuta's Adolescence Syndrome: the chest wounds opening up whenever Shoko-san appears—because two of Sakuta Azusagawa's hearts cannot exist in the world at the same time. At the same time, this caused tension in Sakuta and Mai's relationship, but later Sakuta realized he ultimately couldn't let go of Mai and decided to give up on saving Shoko. On the day of the date, Sakuta saw Shoko-chan still fighting in the ICU, and his resolve wavered again. On his way to the date, he realized Shoko-san's date proposal was actually to keep him away from the accident site. Finally, Sakuta resolved to accept his fate to save Shoko, but was pushed away by Mai, and the worldline changed. |

In this worldline, Mai died in the car accident, and her heart was transplanted to Shoko-chan. Shoko-san in the α-1 worldline stated she observed a future where she married Sakuta in college and changed her name to Shoko Azusagawa. However, Shoko-san from the future in the α-1 worldline still felt Sakuta had not found true happiness, so she helped a grief-stricken Sakuta use his Adolescence Syndrome to return to the past, sacrificing herself to exchange for Sakuta and Mai's happiness. Returning to the day of the accident, Sakuta pushed his past self away at the scene, and the worldline changed once again. |

In this worldline, Shoko lost her "future possibilities" because no suitable heart donor was found, and thus Shoko-san ceased to exist. Shoko-chan in the ICU received future memories of the previous two worldlines in her dreams[4]., On her deathbed, she hoped Sakuta would not worry about her and decided to create a world where the two of them never met. Finally, Shoko passed away on New Year's Day. These future memories enabled her fourth-grade self to complete her "Future Dreams" assignment, and the worldline changed for the third time. |

In this world line, because Shoko's Puberty Syndrome did not occur, Sakuta never met Shoko, but after his younger sister lost her memories, he had a dream in which an unnamed girl from Minegahara High School changed his outlook on life. Meanwhile, influenced by subconscious memories from other world lines, the middle-school-aged Mai starred in a movie filmed at Shichirigahama Beach about a middle school girl with congenital heart disease, which became a massive success. This indirectly led to increased social awareness of the disease, allowing young Shoko to receive a heart transplant from a kind donor and survive.Volume 14 reveals that this person is none other than Touko Kirishima. At the end of Volume 7, Shoko, Sakuta, and Mai are at Shichirigahama BeachReunion, Sakuta called out Shoko's name based on a physical instinct, and Shoko also called him "Sakuta-san." It should be noted that Shoko retains memories from the previous timelines. The stories after Volume 7 of the novel all take place based on this world line. In Volume 8, she moves to Okinawa to reduce the physical burden on her bodyExiled to the frontier by Kamoshida。 In Volume 9, Shoko sends a letter to Sakuta along with a vibrant photo of herself wearing a white dress. At the end of the letter, she tells Sakuta that she will continue to write to him. In Volume 11, she sends another letter and photo to Sakuta; by this time, young Shoko has grown to look more and more like the "older Shoko" who was Sakuta's first love. In Volume 14, because she was admitted to Minegahara High School, she once again In Volume 15, Shoko serves as the president of the Biology Club at Minegahara High School, and she is a student of Sakuta, who is serving as a student teacher. |
